Connecting with you
We work closely with you, and where appropriate, your carers or family members, to understand your challenges and develop an action plan tailored to your needs. This includes assessments at your home and/or in your local community.
Providing recommendations
Based on our assessments, we provide practical solutions to promote independence, improve safety; and make daily living easier.
Building capacity
We work with you to strengthen your ability to manage daily tasks.
Assist with enhancing your ability
to enable you to stay at home. We build on your strengths and capacity and focus on your goals to promote greater independence and autonomy.
Provide you with assistive technology (AT)
This is an item or piece of equipment providing you more independence. Items include
Toileting aids such as a frame over the toilet
Bathing aids such as a shower chair or bathboard
Small kitchen aids such as large-handled cutlery
Bedroom aids such as a bedstick or sheet to make turning over in bed easier
Arrange home modifications (HM)
which are changes to your home to create safer environments. Examples include
